Homegrown Nutrition accepts most major insurance for nutrition services. Many insurance companies cover nutrition sessions for preventive care, even without a formal medical diagnosis.

Our goal is to get your insurance to cover at 100% if possible. This form will help us to do that!     *This includes Medicare Advantage from private insurers. Please note that Medicare Part B only covers renal disease (not on dialysis) and diabetes visits.

  • This means we are an out-of-network provider. If this is the case, you will need to pay for the session at the time of service via cash, check, or credit/debit card. However, Homegrown Nutrition can provide you with a “Superbill” and/or an itemized invoice you can submit to your insurance company for reimbursement. Reimbursement is based on your policy. Please call your insurance company’s member services number to verify how reimbursement works under your current insurance policy.

    Please note that payment is due at time of service. Initial nutrition visits are $220-330 and each nutrition follow-up visit are $100-200. Homegrown Nutrition accepts cash, check and all major credit cards.
